If you don't find a replacement you may be able to get a short reducer. Plumbing supply shops have more to choose from. As well you may be able to access this from the back side of the wall so you don't have to replace tile. again i stress plumbing supply shops. If it is available they can find it. If not the back side is next,or last option remove tiles. If you are somewhat handy the job is not that hard.ADDED COMMENT If anything is out there to do this a plumbing supply shop will be able to get it for you. Buying a bushing sounds good but try to find one.

Get a 3/4" - 1" adaptor. If you don't want to damage the bathroom wall, see if you can get access from the other side , behind the faucet.

Plan on taking the whole wall apart. Your best bet is to buy a whole new faucet and all of the problems will go away.. I don't know of any way to re-connect the lever.

They may not make them anymore but the Internet is a wonderful thing. I found a website:
They deal with old fixtures that are no longer being made and you should be able to find a replacement. Good luck.
